WoundWise, operated by SentientBio LLC, is a mobile wound care company bringing specialized, in-home and facility-based wound treatment directly to patients across our service area. We pair advanced wound care therapies with a patient-first, travel-to-you model — meeting people where they are in their homes, assisted living communities, and skilled nursing facilities. As we grow, we're looking for an experienced, independent-minded Nurse Practitioner to join our clinical team.
Position Summary
As a Mobile Wound Care Nurse Practitioner, you'll provide specialized treatment for chronic and acute wounds across a variety of care settings. You'll travel to patients and partner facilities to perform wound assessments, dressing changes, and advanced therapies — including ultrasounic debridgement treatment, Skin Substitutes & WoundVac’s — while monitoring infection control, collaborating with care teams, and educating patients and families to drive better healing outcomes. This is a hands-on, autonomous role ideal for a clinician who thrives in a mobile, fast-paced environment.
Requirements
Conduct comprehensive wound assessments and ongoing evaluations of wound progress
Perform wound cleansing, dressing changes, and wound management treatments
Provide debridement services, only when properly licensed, trained, and certified to do so
Apply advanced wound care products and therapies, including ultrasound treatment, when clinically appropriate
Administer IV therapy as clinically indicated and within scope of practice
Travel to patient homes, assisted living communities, skilled nursing facilities, and other care settings to provide hands-on care
Educate patients, caregivers, and family members on treatment plans, pressure relief, dressing changes, infection prevention, and related wound care protocols
Monitor wounds for signs of infection, drainage, odor, deterioration, and other complications
Document all patient encounters, care provided, observations, and treatment recommendations in the applicable electronic medical record (EMR) system
Collaborate with physicians, home health providers, facility staff, and other interdisciplinary team members
Use EMR and related technology for remote assessment, monitoring, and follow-up as needed
